It turned out that there was a bug in the program getstar that caused it to ignore the -minmag flag. This caused brighter stars to be used than intended. Apparently in the skycells in question the bright stars caused the grid search to . Fixing getstar caused a better set of stars to be selected and astrometry to succeed.
